Frequently Asked Questions

You will receive free shipping on all prescribed medications filled by one of our partnering pharmacies. If you prefer to use a local or third-party pharmacy you will be responsible for the cost of the medication.

If you have selected our Monthly Plan, your membership will be automatically renewed at the end of the billing cycle. If you have selected our 12 Week Plan, one of our representatives will reach out to you with how to continue treatment. 

You may cancel your subscription anytime by contacting our customer support team at support@joinelevate.com with the subject line “Cancel my account” or calling (833) 669-2112 (only applies to monthly plans, 12 week plans are one-time payments therefore cannot be cancelled). Please note, that not completing a refill form or not finishing the sign-up process does not result in cancellation.

Elevate Health does not currently offer a trial membership.

We use third-party pharmacy fulfillment partners to ship most prescribed medications directly to our members.

After signing up you will be asked to fill out a medical intake form to help determine if you are eligible for a prescription. Prescription medication isn’t right for everyone. Certain pre-existing conditions, medical history, health data, and/or conflicting medications could prevent a weight loss medication from working safely and effectively. If, after you fill out our comprehensive intake form, you are ineligible for an Elevate Health and Wellness program, we will refund all amounts you paid at initiation and cancel your subscription. 

Any refunds processed by our Support Team should be reflected in your account within 3-10 business days, depending on your credit card company. Once we’ve processed your refund, you will need to follow up with your bank for any updates on when they will release the funds.

Yes. To prioritize member health and safety, the Rx Path is for qualifying members with a BMI ≥30 (or ≥27 with certain comorbidities).

Please note the cost of labs is not included in your membership fee.

Medication is usually delivered within 7-14 business days once you have been approved.
